How do I decide if hormone and longevity medicine is right for me?
Often clients approach us at EVOKE with symptoms including fatigue, brain fog, decreased exercise capacity, sleep disturbances, hot flashes, weight gain, sexual dysfunction. We provide a free 15min consultation to review symptoms and provide information about our services that can help you to decide if hormone optimization feels right for you
How am I cared for at EVOKE?
Your health care provider will design a unique care plan for you based on your lifestyle, needs, goals, and capacity. They will work with you to come up with a plan that fits your busy life and follow up closely with you to ensure that you are satisfied with your care plan and symptom management.
Patient identity is verified prior to each consultation. During intake, patients provide identifying information such as name, date of birth, and Personal Health Number (PHN). Identity and location are confirmed at the beginning of the telemedicine consultation to ensure care is provided within the practitioner’s licensed jurisdiction
What happens at my first and follow up visits?
Every plan has a comprehensive review where we learn your story- your symptoms, challenges, concerns, and your goals. We review your past medical history and your blood work to ensure that bioidentical hormone therapy is safe for you. Follow up is often with a review of blood test results and symptoms, hormones are adjusted at this time as needed. Frequency of follow up is determined by your membership, your wants and needs and can be as often as weekly. On average, follow up initially is every 6-8 weeks and as your prescriptions are stabilized and optimized follow up is every 2-3 months. We are available to you between these scheduled visits on an as needed basis to answer any questions or concerns.
When will I start feeling better?
Depending on your concerns, you may start feeling better immediately. Sleep for example, often improves quickly. Metabolic changes are seen in the first 3-6 months and the greatest changes occur after 9-12 months of hormone therapy and lifestyle optimization.
Why do I need to do blood tests and follow ups?
Hormone balance is unique to each person and while we titrate bioidentical hormones to each persons individual symptoms, we also ensure that their levels are within optimal ranges with blood tests. Each persons care plan and follow up may look different depending on their individual needs and symptoms. Once optimized levels are achieved and symptoms resolved the need for blood testing decreases. This is usually achieved after the first 9 months of care.
Can you explain your billing model?
We understand that you may have questions about the initial $700 onboarding fee — especially if you’re comparing care models or telehealth platforms.
At Evoke, this fee is not simply a charge for a “first visit.”
It functions as a deposit toward the comprehensive clinical work required to design your personalized care plan.
Our model is built around depth, strategy, and longitudinal support — not quick prescribing or one-size-fits-all protocols.
We follow up every 6- 8weeks and are available anytime you need in between- because access to close follow up is key to getting the most out of your HRT.
What does the onboarding fee include?
Your onboarding investment covers the full foundation of your care:
Comprehensive 60-minute intake consultation
Detailed blood work review and interpretation
Personalized care plan design and clinical strategy
A second 60-minute follow-up visit
Education and teaching around your results and treatment plan
Prescription initiation and ongoing management
This process allows us to move beyond symptom management and build a truly individualized hormone and metabolic strategy.
Why is there a monthly fee?
The ongoing monthly fee is structured as a payment plan to make this level of care more financially accessible over time — rather than placing the full cost of onboarding, follow-up, and care design upfront.

How is Evoke different from telehealth platforms?
Evoke is intentionally designed to operate differently from high-volume telehealth models.
We do not practice:
Protocol-based prescribing
One-size-fits-all hormone dosing
Minimal follow-up care
Instead, we focus on:
Deep lab analysis
Symptom-lab correlation
Personalized treatment design
Close clinical follow-up
Prescriptions issued only after consultation
Controlled substances prescribed only when medically appropriate

Many of our patients initially began care through large telehealth companies and later transitioned to Evoke seeking more thorough assessment, individualized planning, and ongoing support.
Are your services covered by my benefits?
Evoke is a private medical clinic which means that care is not covered by MSP or extended benefits. Blood tests are covered by MSP.
If you have a Health Spending Account through your employer, costs can be submitted.
Medications are directly billed to the patient by pharmacy and coverage is dictated by individual benefits and health care plans
