First visit is a comprehensive assessment and consultation where we get to know your personal life experience, history, symptoms and goals. A customized blood requisition design specific to your symptoms, history, and goals is created.
Second visit is presents a tailored care plan specific to your blood work results, personal needs and goals with hormone and lifestyle prescriptions.
Ongoing close follow up every 6 weeks or more initially and then every 2-3 three months ongoing
Each follow up includes blood work as needed, symptom review, care plan fine- tuning, prescription adjustments and refills as needed. Because HRT is not a one size fits all.

We offer comprehensive weight-loss support using GLP-1–based medications paired with personalized lifestyle optimization care and support.
Our Weight Loss Care Plan includes ongoing medical guidance, monitoring, and follow-up visits every 4-6 weeks during the initial phase to ensure safe progress and medication tolerance and sustainable results.
Our approach focuses on sustainable habit change, improved metabolic health, and close clinical oversight and prescribing—so you feel supported, informed, and confident throughout your weight-loss journey.
Our Weight Loss Care is a separate program from Evoke Hormone Care however may be blended together if requested

Health Care Coverage
Hormone optimization is not covered by extended benefits.
If you have a Health Spending Account (HSA) through your insurance, our consultation cost can be 100% covered.
Otherwise our costs are considered eligible medical expenses that can be 100% deducted against your income tax - please check with your tax professional for details.
Insurance: specific coverage varies by provider and plan. We recommend you check with your insurance company to confirm your personal coverage. All bloodwork is generally covered by your provincial health card.

Your journey to our personalized hormone care begins with a virtual consult with your clinician. Here we learn about your medical history, symptoms, lifestyle so that we can design your unique care plan specific to your symptoms and goals.
We complete a preventive health assessment and safety profile including cancer screening, osteoporosis, metabolic and cardiovascular health.
Laboratory requisitions are designed for your unique needs
Each visit includes a symptoms review, blood work analysis as needed, prescription optimization as needed specific to your symptoms, goals, and hormone responses.
