First visit is a comprehensive assessment and consultation where we get to know you- past medical history, family history, risk assessment, life experience and context, medication review, symptoms and goals. A customized blood requisition specific to your symptoms, history, and goals is designed. Hormone education woven throughout visit

Second visit presents a tailored care plan developed specifically for you- to your blood work results, personal needs and goals. This hour long appointment includes blood work review so that you what we see, a hormone prescription recommendation if medically indicated, and a lifestyle prescription with education around optimizing diet and exercise specific to you. References and written risks/benefits/side effects are provided for all recommendations made.

Ongoing close follow up every 6 weeks and then every 2-3 three months ongoing. Access to your licensed NP between appointments as needed at no additional cost- for questions, review, support, refills as needed.
Each follow up includes blood work as needed, symptom review, care plan fine- tuning, prescription adjustments as needed and if indicated, and refills. Tailored care because HRT is not a one size fits all.

We offer comprehensive weight-loss designed with personalized lifestyle optimization care and support.
Our Weight Loss Care Plan includes ongoing medical guidance by licensed NP, monitoring, and follow-up visits every 4-6 weeks during the initial phase to ensure safe progress , medication tolerance and sustainable results. Medications are prescribed with close consideration and if medically indicated.
Our approach focuses on sustainable habit change, improved metabolic health, and close clinical oversight and prescribing—so you feel supported, informed, and confident throughout your weight-loss journey.
Our Weight Loss Care is a separate program from Evoke Hormone Care however may be blended together if requested

Health Care Coverage
Hormone optimization is not covered by extended benefits.
If you have a Health Spending Account (HSA) through your insurance, our consultation cost can be 100% covered.
Otherwise our costs are considered eligible medical expenses that can be 100% deducted against your income tax - please check with your tax professional for details.
Insurance: specific coverage varies by provider and plan. We recommend you check with your insurance company to confirm your personal coverage. All bloodwork is generally covered by your provincial health card.

Your Care
Your care with Evoke HRT begins with a telemedicine consultation with a licensed healthcare practitioner. During this visit, the practitioner reviews your medical history, symptoms, medications, and lifestyle factors to understand your health concerns and goals.
A preventive health and safety assessment may include review of cancer screening history, bone health, metabolic health, and cardiovascular risk factors. Laboratory testing may be requested when clinically appropriate to support evaluation and treatment planning.
Based on the clinical assessment, the practitioner may recommend lifestyle guidance, monitoring, or medical treatment when appropriate. If prescription medications are clinically indicated, prescriptions are transmitted to licensed pharmacies for dispensing.
Follow-up visits include review of symptoms, laboratory results when available, and ongoing monitoring to ensure treatment remains safe and appropriate.
