How Telemedicine Care Works at Evoke BHRT
Evoke BHRT provides medical care through secure telemedicine consultations with licensed healthcare practitioners. Our care process is designed to ensure that patients receive safe, individualized medical evaluation and treatment when clinically appropriate.
1. Secure Intake and Medical History
Patients begin by completing secure intake forms that collect medical history, symptoms, medications, and relevant health information.
2. Identity Verification and Telemedicine Consent
Before the consultation begins, the patient’s identity is verified using their name, date of birth, and Personal Health Number (PHN). Patients review and consent to receiving care via telemedicine prior to the appointment.
3. Telemedicine Consultation
A licensed healthcare practitioner conducts a comprehensive medical consultation, reviewing the patient’s health history, symptoms, medications, and health goals.
4. Clinical Evaluation and Testing
When clinically indicated, laboratory testing or additional medical information may be requested to support clinical decision-making.
5. Individualized Treatment Plan
Based on the clinical assessment, the practitioner may recommend lifestyle guidance, monitoring strategies, or medical treatment when appropriate.
6. Prescriptions and Care Coordination
If prescription medications are clinically indicated, prescriptions are transmitted to licensed pharmacies for dispensing. Evoke BHRT does not sell or dispense medications directly.
7. Follow-Up and Monitoring
Patients receive ongoing monitoring and follow-up care to ensure that treatment remains safe and appropriate over time.
