Membership Cancellation, Billing & Prescribing Liability Policy
1. Membership Care Model
Evoke provides longitudinal, membership-based medical care. Enrollment includes ongoing clinical oversight, treatment planning, care coordination, and prescription management. Membership fees reflect access to continuous medical services rather than payment for individual visits alone.
2. 30-Day Written Notice of Cancellation
Members are required to provide a minimum of thirty (30) days written notice to cancel their membership.
Written notice must be submitted via secure patient portal or email to the clinic’s designated contact address.
The 30-day notice period begins on the date written notice is received.
3. Prescribing & Controlled Substance Liability
The 30-day notice requirement is mandatory due to the medical and legal responsibilities associated with active patient care, including but not limited to:
Prescriptions issued under the provider’s medical license
Monitoring obligations for hormone therapies and adjunct medications
Prescribing and oversight of medications classified as controlled substances (where applicable)
Clinical duty to ensure safe continuation, tapering, discontinuation, or transfer of care
This notice period ensures compliance with federal and provincial prescribing regulations, professional college standards, and controlled substance monitoring requirements.
Membership clinical oversight remains active throughout the notice period.
4. Billing During Notice Period
Membership fees will continue to be billed during the 30-day notice window.
Billing during this period reflects:
Ongoing prescribing liability
Medical record maintenance
Care coordination
Patient access to clinical communication
Safe transition or discharge planning
Failure to utilize services during the notice period does not exempt the member from billing obligations
5. Refund & Charge Reversal Policy
By enrolling in membership, patients authorize recurring billing in accordance with the care agreement.
Accordingly:
Membership fees are non-refundable once processed
Previously billed or paid fees are not eligible for reversal or chargeback
Partial month refunds are not issued
Administrative or onboarding fees are non-refundable
This policy reflects the allocation of clinical, administrative, and prescribing resources upon enrollment.
6. Early Termination at Clinic Discretion
Evoke reserves the right, at its sole discretion, to waive or modify the 30-day notice requirement as a professional courtesy. Such modification does not create entitlement to fee refunds or reversals.
7. Immediate Termination by Clinic
The clinic reserves the right to terminate membership immediately in cases including, but not limited to:
Abuse of staff
Misuse or diversion risk of prescribed medications
Non-compliance with treatment agreements
Failure to adhere to controlled substance policies
Fraudulent payment disputes or chargebacks
In such cases, patients remain financially responsible for services rendered and prescriptions issued up to the termination date.
8. Acknowledgment of Agreement
Enrollment in membership constitutes acknowledgment and acceptance of this Cancellation, Billing, and Prescribing Liability Policy.
9. Telemedicine: Evoke BHRT provides healthcare services through telemedicine consultations conducted by licensed healthcare practitioners. Telemedicine allows patients to receive medical care remotely using secure communication technology. While telemedicine can improve access to care, certain medical conditions may require in-person evaluation or physical examination. If telemedicine is not appropriate for a patient’s needs, the practitioner may recommend in-person care

Patients are responsible for providing accurate medical information, including symptoms, medical history, and current medications, and for completing recommended laboratory testing or follow-up when requested.
Evoke BHRT does not provide emergency medical services. Patients experiencing a medical emergency should contact local emergency services or go to the nearest emergency department.
