Evoke BHRT is committed to protecting the privacy and security of patient health information. The clinic maintains administrative, physical, and technical safeguards designed to protect the confidentiality, integrity, and availability of protected health information (PHI). Patient information is collected, used, stored, and disclosed only as necessary to provide healthcare services and in accordance with applicable healthcare privacy regulations.

All electronic medical records and patient communications are maintained using secure, encrypted systems that are designed to be HIPAA-compliant. Access to patient information is restricted to authorized healthcare practitioners and staff members who require access to perform their duties and provide patient care. Secure authentication, role-based access controls, and encrypted data transmission are used to protect patient information from unauthorized access.

Evoke BHRT uses secure telemedicine and electronic medical record platforms to conduct consultations, store medical records, and manage patient information. These systems utilize industry-standard security protocols to ensure that patient data is protected during transmission and storage.

Patient information is not sold or shared with third parties except as required for healthcare operations, such as transmitting prescriptions to licensed pharmacies, coordinating laboratory testing, or when required by law. Patients have the right to access their medical records and request corrections in accordance with applicable privacy regulations.

To ensure patient safety and compliance with telemedicine standards, patient identity is verified prior to each consultation. Patients provide their full legal name, date of birth, address, and Personal Health Number (PHN) during the intake process. At the beginning of the telemedicine visit, the practitioner confirms the patient’s identity and verifies the patient’s physical location to ensure care is provided within the practitioner’s licensed jurisdiction
